Unless you're old like moi, very few people know that Philadelphia guard Hal Greer used to jump shoot his attempts at the foul line. In his 15 NBA seasons, he had an 80% accuracy rate. This clip has video of him doing so but just one attempt.
Also, Don Nelson's free throw form was downright ugly but he shot 76% over his 14 years:
